Its greatest strength isn’t that it teaches more pathology than other MSK resources. Instead, it teaches how to systematically interpret musculoskeletal MRI. Throughout the fellowship, Dr. Christoph Agten emphasizes developing a repeatable search pattern that helps you approach every examination with greater consistency, confidence, and efficiency.
That was the biggest change I noticed in my own practice.
Rather than mentally jumping between structures during complex MRI examinations, I developed a more organized workflow that I continue to use every day. One piece of advice I’d give anyone considering the fellowship is simple:
Spend an hour watching Dr. Agten’s free YouTube videos first.
Every educator has a different teaching style, and the fellowship closely mirrors the way he teaches on YouTube. If you enjoy his free content, there’s a good chance you’ll enjoy the fellowship as well.
I actually see this as one of the program’s biggest advantages. Very few educational programs allow you to experience so much of the instructor’s teaching before making a purchasing decision.
Agten Radiology Virtual MSK Fellowship Review: My Experience
I purchased the Virtual MSK Fellowship because I wanted to become a better musculoskeletal radiologist—not simply earn CME credits.
Overall, I believe the fellowship accomplished exactly that.
The biggest lesson I took away wasn’t learning hundreds of new diagnoses.
It was learning a better process.
Before taking the fellowship, I occasionally found myself approaching more difficult MRI examinations by jumping between structures as I searched for abnormalities. Dr. Agten’s systematic approach helped me develop a more consistent workflow that I now use on nearly every musculoskeletal MRI.

Being an excellent radiologist doesn’t automatically make someone an excellent educator.
Fortunately, Dr. Agten is both.

That teaching style made it much easier for me to retain what I learned.

There’s nothing wrong with that, but this program has a different goal.

Areas for Improvement
No educational program is perfect, and I think it’s important to discuss both the strengths and weaknesses.
Fortunately, I found relatively few limitations.
Limited Coverage of Musculoskeletal Tumors
This was my biggest criticism of the fellowship.
The curriculum provides excellent education on sports injuries, degenerative disease, trauma, postoperative imaging, tendon pathology, ligament injuries, and many of the conditions radiologists encounter every day.
However, bone tumors, soft tissue tumors, and tumor-like lesions receive little to no dedicated coverage.
I don’t necessarily think this was the wrong decision. The fellowship appears intentionally designed around the pathology most radiologists interpret on a daily basis.
Still, I would have appreciated additional education on musculoskeletal tumors.
If that’s an area you specifically want to strengthen, you’ll likely need supplemental educational resources.
It’s Not Designed for Quick CME
If your only goal is earning CME credits as quickly as possible, this probably isn’t the best choice.
The lectures are comprehensive, detailed, and require a meaningful time commitment.
Personally, I viewed that as one of the program’s strengths because I wanted education that improved my clinical practice—not simply another way to fulfill my annual CME requirement.
Who Should Consider the Fellowship?
I think the Virtual MSK Fellowship provides the greatest value for radiologists who interpret musculoskeletal MRI on a regular basis.
Residents and Fellows
If you’re still in training, the fellowship provides an excellent foundation for developing a systematic approach to MRI interpretation early in your career.
Early-Career Attendings
The transition to independent practice can be challenging. Having a repeatable workflow for interpreting musculoskeletal MRI can improve both confidence and efficiency as your workload increases.
General Radiologists
If musculoskeletal MRI is a regular part of your practice, I think you’ll gain a great deal from this program. The fellowship focuses on practical skills that can be applied immediately in everyday clinical practice.
Fellowship-Trained MSK Radiologists
Even after completing an MSK fellowship, I still found the program worthwhile.
It didn’t completely change the way I read MRI, but it refined my workflow, reinforced important concepts, and helped me become more systematic and efficient.
Who Might Not Benefit as Much?
The Virtual MSK Fellowship isn’t the right fit for everyone.
If you’re simply looking for the fastest way to earn CME credits, there are quicker and less expensive options available.
Similarly, if you only occasionally interpret musculoskeletal MRI, you may not receive the full value of the fellowship. Its greatest strength is helping radiologists develop a repeatable workflow that improves confidence and efficiency over time. Those benefits are most apparent when you regularly read MSK MRI and can consistently apply what you’ve learned.
Finally, if your primary educational goal is musculoskeletal tumors and tumor-like lesions, I recommend supplementing the fellowship with additional educational resources, as those topics receive minimal dedicated coverage.
CME Credits
In addition to the educational content, the Virtual MSK Fellowship offers the opportunity to earn continuing medical education (CME) credits.
Upon successful completion of the program, participants are eligible to receive up to 50 European CME credits (EACCME®).
For physicians practicing in the United States, these credits can be converted to AMA PRA Category 1 Credits™ through the appropriate accreditation process.
One important point to be aware of is that there is an additional fee to convert the European CME credits into AMA PRA Category 1 Credits™. Since accreditation policies and fees can change, I recommend reviewing the current requirements before enrolling if U.S. CME credit is important to you.
For me, the CME credits were a bonus. The real value of the fellowship was becoming a better musculoskeletal MRI reader.

I’ve used several online MSK educational resources over the years, and each has its own strengths. Rather than asking which one is “best,” I think it’s more helpful to ask which one best fits your learning goals.

These programs serve different purposes.
Medality offers a broad catalog covering multiple radiology subspecialties with excellent CME options. If you’re looking for education across body imaging, neuroradiology, emergency radiology, pediatric radiology, and more, Medality is hard to beat.
The Virtual MSK Fellowship, on the other hand, is entirely focused on musculoskeletal MRI. Instead of offering broad coverage, it dives deeply into one subspecialty with the goal of improving how you approach MRI interpretation.
Choose Medality if:
- You want education across multiple radiology subspecialties.
- Your primary goal is earning CME in a variety of topics.
- You prefer shorter, standalone lectures.
Choose Agten if:
- You read musculoskeletal MRI regularly.
- You want to improve your workflow and efficiency.

Agten Radiology vs. CaseStacks
Both are excellent educational resources, but I think they complement each other rather than compete directly.
CaseStacks excels at exposing you to a wide variety of real cases. It’s fantastic for pattern recognition, board preparation, and expanding your differential diagnosis.
The Virtual MSK Fellowship spends more time teaching a systematic process for interpreting MRI.
If CaseStacks helps answer the question, “What is this diagnosis?”
Agten helps answer, “How should I approach this MRI?”
Personally, I think many radiologists would benefit from using both resources because they strengthen different aspects of MSK education.
Agten Radiology vs. Radiopaedia
Radiopaedia is one of the best free radiology references available.
I use it regularly.
However, it’s designed as a reference library—not a structured curriculum.
If I encounter an unfamiliar diagnosis, Radiopaedia is often one of the first places I look.
If my goal is to systematically improve my musculoskeletal MRI interpretation over time, I’d choose the Virtual MSK Fellowship.
The two resources serve different purposes and complement each other well.
Final Verdict
After completing the Agten Radiology Virtual MSK Fellowship, I can confidently say it was one of the most valuable educational programs I’ve taken as a musculoskeletal radiologist.
Its greatest strength isn’t the number of lectures or the amount of pathology it covers.
It’s the way Dr. Agten teaches you to think.
By emphasizing a systematic approach to MRI interpretation, the fellowship helped me become more organized, more confident, and more efficient in my daily practice. Those improvements have lasted well beyond completing the course.
